You will need to take a clear photo of your state issued ID as well as a photo of a recent proof of address that you have for the address that you currently live in. The proof of address can be as simple as a utility bill, a cable bill, a cell phone bill, a paycheck stub cover... honestly pretty much anything that has a company name, your name and the address that you currently reside at.
